Hospice Referrals

Omega Health Care hospice is recognized leader because of our philosophy that care for others is a mission. If you have a patient or know of a family that needs help to stay in the home or in a facility because of failing health we would like to talk with them. The majority of the time the patient or family does not recognize the benefits of hospice care.

Hospice care allows people to experience quality of life that they would not experience without it.    Some of the benefits are:

• Physicians design plans of care during office visits that are rarely followed in the home. Hospice can ensure that these plans are followed which deliver the best chance of control of symptoms and reduction in unplanned visits to the hospital.
• 24X7X365 access to medical professionals to answer questions, make visits, or make changes to plan of care.
• Spiritual guidance and counseling to go along with physical care can be provided by the hospice chaplains.
• Financial assistance may be available, facility assistance may be available, end of life planning may be available and will be explored by the hospice social worker.
• In home assistance with light cleaning, cooking, showering and other grooming may be elected and provided through home health aides.
• The most important aspect of hospice in many cases is having a professional experienced person come along side the patient or family member to answer question, provide guidance or understanding.
